Professional Chapter Board Roster Form

Each professional chapter must submit a new board roster form to the AMA Support Center by March 1 every year for the coming fiscal year.  See Professional Chapter Reporting Requirements for more details.

If a new member joins the chapter board mid-year or vacates their seat early, the chapter must submit an updated board roster form to the AMA reflecting these changes.

Submit new or updated board roster forms to membersupport@ama.org.  

If you need a copy of your current board roster form to make updates, you can request this from membersupport@ama.org.
